Player Waiver

Please note that the player waiver covers both a complete indoor season as well as the following outdoor season.

All players’ parent/legal guardian must complete and submit a current CMSA liability insurance waiver through your current Member Club. Please contact your Club operations staff to ensure your player has completed the necessary waivers. 

Families are no longer able to complete the waiver through CMSA.
